You can start with one membership and FLEX into another membership. For instance, if you are being followed for weight management, you will benefit from the Premium Membership for the first year. THEN you may want to be monitored quarterly and step into the Wellness Membership.
Dr. Sarah is double board certified by the American Board of Internal Medicine in two specialties, Internal Medicine and Endocrinology, Diabetes and Metabolism. Because of this, she is proud to provide complete care for members, including primary care Internal Medicine services.
Due to the length of our appointments and high demand, we will request a booking fee of $150 for all New Patient Appointments. We will call you the week prior to your schedule appointment to confirm your date/time. Booking fee will be charged 48 hours prior to your appointment. The New Patient booking fee is non-refundable and will be applied to your first visit or membership.
If membership is cancelled and then restarted, this is subject to availability and there is a re-enrollment fee of $125.
